What is a nurse practitioner, and how are they different from a physician?
Nurse practitioners (NPs) are advanced practice clinicians who are licensed to diagnose conditions, order and interpret tests, prescribe medications, and manage both acute and chronic illnesses. Like physicians, NPs provide comprehensive primary care—but with a strong emphasis on whole-person wellness, patient education, and prevention. Our NPs are board-certified and experienced in treating a wide range of health conditions.
Can I choose your practice as my primary care provider (PCP)?
Yes! We are a full-scope primary care practice, and patients can select our nurse practitioners as their PCP. We coordinate care, manage chronic illnesses, handle referrals, and support all your long-term health needs—just as a physician-led practice would.
Do you offer home visits?
Yes. We provide in-home visits for patients who are unable to be seen in a traditional clinic setting, including those with mobility challenges, complex health conditions, or caregiving responsibilities. These visits are designed to ensure continuity of care and reduce the need for unnecessary emergency room or urgent care visits.
Do you manage chronic conditions?
Absolutely. We provide ongoing management for chronic conditions such as diabetes, hypertension, asthma, high cholesterol, and more. Our care plans are individualized and include regular monitoring, education, and medication management, with a strong emphasis on preventing complications.
Can you help with Workers’ Compensation claims?
Yes. We provide medical evaluations, documentation, and follow-up care for patients with work-related injuries or conditions. We work closely with your employer and insurance representatives to ensure your care is coordinated and your recovery is fully supported.

Can you refer me to specialists if I need one?
Yes. When specialty care is needed, we will coordinate referrals and ensure your transition is smooth. We also follow up with you and your specialist to ensure your care remains integrated and aligned with your overall health plan.
What types of patients do you accept?
We’d love to welcome you to our NP-owned primary care practice! We are a family-centered primary care practice focused on adolescent and adult health, and we are pleased to accept new patients beginning at age 14.
To get started, please complete our New Patient Paperwork in full and return it to our office. Once we receive your completed forms, our team will contact you to schedule your first appointment. Most new patient appointments are scheduled within approximately 4–6 weeks of receiving your completed paperwork.
What is your turnaround time for phone calls and communication?
We understand that timely communication is important. When you call our practice and leave a message, you can expect a return call within 48 business hours.
What are your “business hours”?
Monday through Thursday, excluding weekends and federal holidays. Calls received outside these hours will be returned as soon as possible on the next business day(s).
What is the process for getting a prescription refill?
- Contact your pharmacy first — they will send us the electronic refill request
- Plan ahead before weekends, holidays, or extended travel
- Keep follow-up appointments scheduled for chronic medication management
Do I need a referral to see a specialist?

- We gather and submit the necessary medical information to the specialist.
- Insurance Authorization (if required)
- We work with your insurance company to secure approval before your appointment can be scheduled.
