At this time, you may schedule a new patient intake visit
(virtual or in-office).

Insurance Required

  • Please enter active and accurate insurance information when scheduling. We verify all insurance before confirming appointments. If the insurance is inactive or cannot be verified, the appointment cannot be confirmed.
  • After booking, you must also update your Primary Care Provider (PCP) with your insurance plan to North Brookfield Primary Care before your visit.

New Patient Visits Usually Include:

  • Step 1: Intake interview (virtual or in-office) to review history and medications
  • Step 2: In-person physical exam
  • Step 3: Fasting labs when appropriate
